About:	Home Meds RX is an entity of Linden Care Pharmacy.You can view their web site at www.LindenCare.com. We deliver the patients medications to their homes.The medications are packaged in individual blister packs divided by the day for the month.The morning, afternoon,and evening medications are separated by color coded calendar cards which correspond to the time of day the pills are to be taken. We also monitor the course of medications and check with the doctor for renewal.If a doctor's visit is recommended before renewal of the medication we will follow up with the patient or contact person and inform them of the need for a doctor's appointment. We bill the insurance of the patients.They are only responsible for the co-pay.This service is free.This process of personal prescription service helps maintain the individual in good health in the community longer.
